We are proud to announce that from 1st April 2018 City Hospice will be providing community-based care across the whole of Cardiff, taking over the care of patients with the remaining 10 GP practices in the City.
Previously, City Hospice cared for patients registered with 39 of the GP practices in Cardiff, but the Cardiff & Vale University Health Board invited us to take over the care of patients with the remaining 10 GP practices.
