Integrated Palliative Medicine Clinic

We are aiming to continually improve the service we provide at City Hospice. We are very lucky to have a modern and inviting centre at Ty Hosbis with lovely gardens and a dedicated team of volunteers to make people feel welcome, and we wish to make the most of these facilities.

If you are mobile and able to attend, we have started a new Integrated Palliative Medicine Clinic at Ty Hosbis, where in 1-2 hours you will meet one of our consultants Dr James Davies, a clinical nurse specialist, our welfare rights officer Katie and our occupational therapist Rachel. If appropriate, attending this clinic will be offered to you when we first contact you to book a visit.

Coming to Ty Hosbis will be of benefit to you, as you will be able to meet a number of the team members who will help you in one visit.

Before meeting the team, you will be asked to complete a questionnaire of the difficult symptoms and worries you have been suffering with over the past 3 days. This will mean that our team can address the issues most important to you. You will also be asked to give anonymised feedback on what was done well and what can be improved upon, allowing us to make the clinic as useful as it can be for patients and families.

Following your clinic visit, depending on your individual situation, you will either be followed up in the consultant clinic, visited at home by a clinical nurse specialist from City Hospice or discharged if both you and the team feel ongoing input at City Hospice is not needed at that time. However you can be referred again at any point in the future.
